Notice that the woman is barefoot, and thus most likely very poor. It is difficult to discern her expression as she receives the medicine from the doctor. Notice also that she and the children are black, and the doctor is white--although
this fact is not one of the identifying details ("public health doctor,"
"tenant family") mentioned in the photograph's caption.
